The following are required of ALL chaplaincy programs and must be included with application:

1. Copy of valid drivers license
2. 2 passport type photos (clear enough to produce ID) or photo on disk
3. Criminal History record obtained from your local Sheriff Department (or on-line)
4. Up-to-Date Personal Resume
5. Completed Membership Application Form (green form)
6. Completed Membership Form, payment plan (blue form)
