The following are required to be included with application:
1. Application Checklist
2. Completed Membership Payment Form
3. Completed Application for Membership Form
4. National Criminal History report obtained from www.OC Detectives.net or FBI.gov
5. Pastor or Overseer recommendation (or an Ordained Minister for applicants who are a pastor)
6. Payment of One Time $250 Application fee and $250 Annual Dues ** ($500 Total)
7. Up-to-Date Personal Resume
8. List Community Service experiences
9. Copy of Certificate of Completion from Basic Chaplaincy Course
10. Recommendation letters from two (2) business people
11. Copy of valid driver’s license
12. 2 passport type photos (clear enough to produce ID) or photo on disk (or taken at class?)
13. Copy of Ministerial License and/or Ordination (if applicant is Licensed or Ordained by another organization)
14. Recommendation of a Chaplain (helpful if you know one)
15. Location and date of seminar attended.
